Orders Information / Request Form

Bookstores and distributors may use this form (see below) to place orders of multiple copies of any of our products (see Publications). We do offer substantial discounts. If this is your first order, be sure to ask us for the discount rate. Please include the product name and quantity for each item.

We apologize that we are not able to offer on-line credit card ordering. Please do not send any private information (such as credit cards, etc.) in the e-mail form below. We will send an invoice with the order when it ships.
